Patient Complaint Process
Valence Medical Imaging is committed to providing fast, efficient and quality care to our patients. We ask that you contact us, should you have any questions and/or concerns regarding the service or care you received at any of our facilities.
Please use the following methods to provide your feedback:
Online
Feedback may be provided through the VMI website under the “PATIENT” tab, select “PATIENT SURVEY” from drop down menu. Feedback sent through the website will be directed to the location Supervisor or designate and a copy will be sent to the manager of operations.
By Telephone
Telephone feedback can be directed to location Employees to be addressed immediately where possible or VMI Head Office if complaint requires further investigation and will be reviewed for action.
In Person
Feedback in person should be directed to location Employees to be addressed immediately where possible or to the location Supervisor or designate.
